For grounding a single ground plane is used for both RF and Digital Grounding.
For Antenna it is recommend that a 3 component L type pad with series 6.8pF blocker cap be used between RF output and antenna matching L network. This
allows for versatility in the design such that the match to the antenna maybe improved and/or the blocking margin increased by use a LC filter.
